Office Hours

Please call (386) 586-1850 to schedule your visit in advance.  Appoints may be made Monday through Friday, 9:00am - 5:00pm.  The office is closed on weekends and major holidays.  We are located within Florida Hospital Flagler.  Follow the directions to the Physician Offices, Suite 2815.  See below for directions to our office.

 

Emergencies

One of our doctors is always on call (24 hours a day, seven days a week) for emergencies.  If you need to see or speak with one of the doctors outside of office hours, please call the office number (386) 586-1850 to be connected with the surgeon on call, or go directly to the Florida Hospital Emergency Room.

 

Prescriptions

Approved prescriptions may be filled over the phone at the pharmacy of your choice during regular office hours.

 

Fees

The Surgeon’s fees vary according to the procedure performed, and we are happy to answer any questions for you.  After major surgery, there is no additional charge for routine follow-up visits.  Hospital fees and other services such as lab, radiology, or anesthesiology are billed by those providers or physicians.  If you need to obtain information on these charges, we can help direct you.

 

Insurance

Please provide us with your insurance information so we may bill your insurance carrier directly.  If your policy requires a co-pay, you are responsible for paying this amount at the time of service.  Please make sure your primary physician sends any needed referral.  If we do not participate with your insurance carrier, you will be responsible for any amount not covered by your insurance.

 

What to Bring

Having the following ready for your appointment will help us to help you better:

  •  Completed Patient Information Form         

  •  Insurance Card and information

  •  Any pertinent X-rays and reports

    • Mammograms if done at facility other than Florida Hospital

  • A list of your medications with doses

  •  A list of questions you may have

Cancellations

If you are unable to keep an appointment, we ask that you kindly notify us as soon as possible.  This courtesy, on your part, will make it possible to give your appointment to another patient.
